Evaluation rubrics for high school tryouts?

Looking for evaluation rubrics for high school tryouts. This will be my first year where I will have to cut players and I will need evidence to why. Any help would be appreciated.

I hope this helps - I've emailed an evaluation that we used at club and National team level to Sportplan and they will include a link to download it below.
